Soul Care Counseling Agreement (Minor)

We offer Soul Care at KLM to provide a safe and confidential journey into healing. By combining proven biblical and pastoral counseling principles, Soul Care provides faith-based counsel and prayer that helps restore the whole person—spirit, mind, and body. As we facilitate intimate encounters with Jesus, He sets people free. Free from emotional pain. Free from depression. Free from behaviors that lead to broken relationships. Free from trauma. Free from shame.

Life change is a process. God wants to peel back the layers of pain and trauma that have defined and limited you, bringing awareness and ongoing transformation as you commit to the journey. Many of the things that keep us in bondage are hidden deep beneath the surface. To live free and whole, we must first identify the root cause. During our Soul Care sessions, we will help your child uncover the root of their pain and position them to experience change in the presence of Jesus.

Release, Consent, and Waiver of Liability

Soul Care Counseling Ministry is not a psychological counseling service, nor is it intended to be. I understand that the team members are not licensed or insured psychologists or psychiatrists. They are trained in using the Word of God and prayer through the power of the Holy Spirit. I am aware that my child’s participation in Soul Care at Kelley Latta Ministries does not replace psychiatric treatment, psychotherapy, therapeutic counseling, or any other form of professional counseling or therapy.

All personal information gathered during Soul Care sessions is confidential and the files are so maintained. I do hereby give permission for the Soul Care provider to consult regarding my child’s care with other members of the ministry as needed. I further acknowledge and understand that any information provided during Soul Care session(s) shall be considered confidential and shall not be disclosed except as required by law.

I hereby release Kelley Latta Ministries, their Board of Directors, staff members, and lay ministers from all personal and corporate liability or responsibility for any present or future claims from myself, my heirs, or assigns. I further release them from all liability for any personal or psychological injury and, hereby, give my permission to review, consult, and advise per their discretion.
